• 2 letters of recommendation with waiver/rating form, preferably 1 academic and 1 professional
• Official transcripts sent from all schools attended
• $75 application fee
• Personal statement
• Resume

Midwifery Application Requirements

For Bachelor/Master and Master of Science in Midwifery programs:

• 3 letters of recommendation with waiver/rating form (at least 2 from academic or professional references; 1 can be a personal reference)
• Official transcripts (for any college-level coursework)

• Resume
• Prerequisite plan
• Interview (if invited, in person)
